Methanol in drinks: the traveller's poisoning that looks like a hangover
Methanol poisoning from counterfeit or home-distilled spirits kills and blinds travellers every year, most visibly in Southeast Asia but also across South Asia, Eastern Europe and East Africa. It is dangerous partly because the early symptoms are indistinguishable from being drunk, and the serious ones arrive many hours later.

The delayed pattern
Methanol itself is relatively harmless; the body converts it into formic acid, and that takes time. So a person can go to bed apparently just drunk and wake 12 to 24 hours later with severe headache, abdominal pain, vomiting, breathlessness, and — the telling sign — blurred vision, a snowstorm-like visual field, or blindness. By then the damage is advanced. Treatment exists and works, but it is time-critical.
Where it comes from
Usually refilled branded bottles, unbranded local spirits, and cheap cocktails in bars where the spirit is bought loose. Mixed drinks hide the taste completely. The specific patterns that recur are free welcome shots, buckets and unlimited-drink promotions, and local rice spirits or arak.
How to avoid it
- Drink only sealed, branded bottles, and watch them being opened in front of you.
- Avoid free shots, buckets, and unlimited-drink deals.
- Be wary of unusually cheap spirits, and of local moonshine, arak and home-distilled liquor.
- Check seals and labels — poor printing, mismatched caps and refilled bottles are the tell.
- Stick to beer, wine from sealed bottles, or soft drinks where you have any doubt.
If it happens
- Get to a hospital immediately if anyone develops visual disturbance, severe headache or breathlessness after drinking — do not wait for it to pass.
- Say the words "possible methanol poisoning" to medical staff; it changes the treatment pathway.
- Treat delayed, worsening symptoms as the emergency, not the initial drunkenness.
- If several people who drank together are ill, tell the hospital — that pattern is diagnostic.
